Special Education/Transition Planning Consultation Services   2011-2012

As a member of the Nassau BOCES Perkins Consortium your school district is entitled to receive free days of transition consulting services. The purpose of this initiative is to increase College, Career and Life Readiness for students with disabilities.

We have been in many districts over the past several years. This year we plan on focusing on state mandated outcomes to assess the effectiveness of the delivery of transition services including best model practices.

Once again we look forward to assisting all districts in the consortium by identifying, developing and enhancing your transition efforts.

District Based Activities:

  • Initial assessment of your district’s current transition level using the Best Practices Model Transition Guide

  • Determine the district’s levels of readiness in order to  increase student outcomes

  • IEP Development –To provide staff development in IEP preparation which proactively addresses the inclusion of indicator 13  into   the document, recognizing the goal of increasing college, career, and life readiness

  • Target transition areas will be identified and recommendations offered
